How This Position Supports Our Vision
The Family Haven Case Manager, in cooperation with the Family & Young Adult Services Manager and Director of Family & Young Adult Services, is accountable for smooth operations of the Family Haven which includes all aspects of shelter and transitional housing services for up to 9 homeless families who seek shelter, including intakes, providing initial referrals for services, ensuring immediate needs are met (like food, toiletries, etc.), maintaining shelter supplies and inventories, explaining shelter expectations and upholding shelter rules and providing wrap-around case management.
This role serves as a member of the Family Haven Alliance and therefore requires compliance with all expectations of that relationship such as participating in case conference meetings twice a month, providing family triage reports to that group, communicating with community diversion specialist, completing mandatory assessments and reports, and maintaining documentation as required. This position is accountable for maintaining accurate physical and electronic files on all clients which includes all case note entries being made within 48 hours of information acquired.

Units do not become available very often, so Harvest House will hold your application for 6 months in case a unit becomes available. The NSP program has strict gross income limits that are determined by HUD. Please see the chart below to ensure that your household meets the income requirements. Questions? Submitting an application does not guarantee housing. Our goal is to serve as many families as possible, but due to limitations, we select those with the highest acuity of need first. We hold applications for 6 months in case an opening fits the needs of each applicant over that time period.
Home Again is a housing program that helps families prepare for long-term housing stability. The best fit for the program is families that have many challenges to housing but are willing to engage with the program to improve their housing situation. Families must be willing to set goals, meet regularly with a case manager, attend life skills classes, and participate in individual housing stability plans.
